Enter the PIN and Serial Number into the space provided to complete the application form;

You're advised to submit a printed copy of completed application form, birth certificate, two(2) passports, and transfer certicate(if transferred) at the school premises;

All completed form application print-out MUST be COLOURED

Please copy the form number for subsequent printing is not possible without form number

Please note only photocopy of the form is to be submitted in ERC along with other attachments.
